Ok, forget Vista for MySQL.  Starting over from XP....

IIS

• Went to Programs control panel and added IIS from the Programs control panel
• Right clicked Computer, chose Manage
• Right clicked on Default Web Site under IIS
• Added index.html to the top of the document list on the Documents tab

Turns out MySQL doesn't just work.

• The MySQL installation requires that User Account Control (UAC) is disabled. If you do not do this, the installation will fail when the system tries to start the service, even if the user allows the change.
• Went to User Accounts control panel and turned off UAC. Had to restart.
